Overview


As a Physical Therapist at Quality Life Services Home Health, you will play a crucial role in helping patients regain their physical functionality and improve their quality of life. You will assess patient needs, develop tailored rehabilitation plans, and guide patients through therapeutic exercises. Your expertise will ensure that patients receive compassionate care aimed at promoting mobility and preventing further injuries. Collaborating with a multidisciplinary team, you will be instrumental in delivering effective rehabilitation services in a home health setting.



Job Responsibilities

  • Evaluate patients' physical therapy needs through comprehensive assessments and examinations.
  • Design and implement individualized treatment plans focused on improving mobility and strength.
  • Educate patients and their families on therapeutic exercises and proper body mechanics.
  • Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
  • Maintain accurate and timely documentation of patient evaluations, treatments, and progress notes.
  • Collaborate with physicians and other healthcare professionals to coordinate care.
  • Provide guidance and supervision to rehabilitation aides and support staff.
  • Stay updated on the latest developments in physical therapy practices and techniques.


Qualifications

  • Master’s degree in Physical Therapy from an accredited program.
  • Current state licensure or eligibility to practice as a Physical Therapist in the relevant jurisdiction.
  • Strong knowledge of physical therapy principles and techniques.
  • Exceptional communication and interpersonal skills to foster relationships with patients and healthcare teams.
  • Demonstrated ability to work independently and manage a caseload effectively.
  • Experience in home health care or a similar setting is preferred.
